The report highlights severe shortcomings in the preparations for the lift, which resulted in an unstable set-up of the cranes and meant that the accident could not have been avoided.

The collapse happened in Alphen aan den Rijn in August 2015 when two mobile construction cranes standing on barges were carrying out a tandem lift of a bridge section from a third barge. Homes and shops were destroyed when they toppled over.

As well as making recommendations for the companies involved, the report also discusses ways to improve risk management on construction projects through actions targeted at government and municipalities.
